Applications and/or Documents Required: Bring along original copies of the following (and be sure pack these in your hand luggage, never in your checked bags):

• one form of identification: Birth certificate, baptism certificate, driver's license, walker's license, passport, military I.D.

• provide date of previous marriage, if applicable
